Current gen: Rayman Origins comes first to mind.

For me, the best game is more likely to be the one with the best narrative, the one that really drew me and had me involved in the story and my character. And the gameplay has to be annoyance free.

This Gen: Red Dead Redemption

All-Time: Metal Gear Solid or Metal Gear Solid 3, one established the other perfected.

This gen: Fallout 3, Mass Effect series. Loved both of them, terrific.

All Time: CHRONO TRIGGER!!!!!!!!!!!! MGS was also great, as were the KOTOR games. The GTA series as well. Half-Life, the Day Of Defeat mod especially, just fantastic. I can't pick a single one, and if I keep typing any longer I'm just going to start throwing out titles left and right. Red Alert 2!!!!!!!
